A decision without a version is fragile
A clearance is only meaningful if it is tied to the exact promise that was evaluated.
If price, scope, SLA, delivery date, liability or evidence changes materially after clearance, the original decision may no longer apply.
The minimum decision record
A useful record should capture the commitment version, the evidence reviewed, key assumptions, expected economics, delivery dependencies, decision state, conditions, authority and the changes that would require re-clearance.
That creates a defensible line from what was proposed to what was authorized.
Why outcomes matter later
The record becomes more valuable when it can be compared with what actually happened: margin, delivery, customer performance and financial outcome.
That is how the organization can learn which kinds of promises consistently create or destroy value.
